How to Fill Out a Form for an Israel Visa: A Practical Guide
Learn how to complete an Israeli visa application, when to use ETA-IL, and why Form 4 and M-4 refer to different matters.

How to fill out an Israel visa application form
There is no single application form for every person travelling to Israel. First check whether your passport requires a visa and identify the correct category, such as a visitor visa (B/2), work visa, or another type. The form and submission process depend on that category and, in many cases, on the Israeli embassy or consulate responsible for your place of residence.
Complete the form using the details exactly as they appear in your passport. Give a clear, truthful purpose of travel, proposed arrival and departure dates, accommodation or host details, and information about your work or studies if requested. Check spelling, passport number, and contact details before submitting. Provide the supporting documents requested for your category, and do not describe paid work as tourism. Follow the relevant Israeli mission’s instructions about signatures, appointments, original documents, and whether the application can be started online.
Choose the right route before applying online
Some travellers who do not need a visitor visa must obtain an electronic travel authorisation, ETA-IL, before travelling. It is not a visa and does not replace one for travellers whose nationality or purpose of travel requires a visa. The requirement for visa-exempt visitors took effect on 1 January 2025. Apply through the official Israeli government service and check the latest eligibility rules before booking or boarding.
If you do need a visa, start with the Israeli mission serving your location and the Israel Population and Immigration Authority (רשות האוכלוסין וההגירה) guidance where relevant. An online form may be only one part of the application; some applicants must still book an appointment or submit documents in person. Requirements vary by nationality, visa type, and mission, so do not rely on a form intended for a different category.
Before you submit
- Confirm whether you need a visa, ETA-IL, or another authorisation.
- Use the current form linked by the relevant Israeli authority or mission.
- Make sure every answer matches your passport and supporting records.
- Prepare evidence requested for your case, such as travel plans, accommodation details, an invitation, or employment documents.
- Keep copies of the submitted form and confirmation, and allow time for questions or additional documents.
Processing time is not guaranteed. The table gives broad planning guidance, not a service commitment; consult the authority handling your application for current instructions.
| Process | What to expect | Where to verify |
|---|---|---|
| ETA-IL | Apply before travel; processing and approval are not guaranteed to be immediate. | Official Israeli ETA-IL service |
| Visitor visa application | Timing varies by mission, application volume, and whether more documents are requested. Allow several weeks where possible. | Israeli embassy or consulate responsible for your location |
| Work or other visa category | May involve additional approvals and supporting documents; timing depends on the category and case. | Relevant Israeli mission and Population and Immigration Authority |
What is Form 4 in Israel?
Form 4 (טופס 4) is a term used in Israeli construction and occupancy procedures, not an Israel visa form. Traditionally, it refers to approval associated with a completed building being suitable for occupancy and, depending on the process, connection to essential utilities. It is not the same as a building permit (היתר בנייה): a permit authorises construction under approved plans, while an occupancy-related approval comes near the end of the project.
Terminology and the exact approval issued can vary between local authorities and under updated procedures. A municipality or local planning and building committee may use a completion or occupancy certificate (אישור אכלוס) or another document in place of, or alongside, the familiar Form 4 wording. For a specific property, confirm the current requirements with the local authority and the project’s permit conditions rather than assuming that every Israeli authority uses the same process.
Typical preparation for an occupancy approval
The owner or project team generally needs to show that the building was completed in line with its permit and that required inspections and professional approvals have been addressed. Depending on the project, the authority may request documents relating to safety, accessibility, utilities, fire protection, engineering, or the safe room (ממ״ד). The precise checklist depends on the building and the local committee. Missing approvals or differences between the permitted plans and the finished work can delay the process, so coordinate the required submissions with the project professionals and local authority.
What is Form M-4, and what is the short form for Israel?
There is no single, universally recognised Israeli visa application called “M-4” that can be treated as a substitute for Form 4. The meaning of M-4 depends on the organisation or document where it appears. If an employer, consulate, municipality, or contractor asks you to complete one, request the issuing body’s full title and instructions before filling it in. Do not confuse an internal form code with a national visa or building approval.
If by “short form for Israel” you mean the country code, the two-letter code is IL and the three-letter code is ISR. Those codes may appear in travel, banking, or online forms; they are not visa categories. For immigration purposes, choose the visa or authorisation that matches your nationality and reason for travel.
CVs and Israeli work authorisation are separate
A CV for an Israeli job application is a hiring document, not a visa form. Prepare it in the language requested by the employer, usually English or Hebrew, and make it concise and easy to scan. Include your contact details, relevant experience, education, skills, and language abilities. Use clear dates and explain international qualifications or job titles where needed. Do not include sensitive personal details unless the employer specifically requires them.
Getting a job offer does not by itself grant permission to work in Israel. Work authorisation is a separate immigration process, and the applicable route depends on the role and your circumstances. Before travelling or starting work, confirm the required visa category and approvals with the employer and the relevant Israeli authorities. Keeping visa applications, building approvals, and job documents separate will help you use the right form for the right purpose.
